Older-person care navigation should consider multiple functional domains and coordinate care rather than assume one isolated problem.

Falls, near-falls, unsteadiness and reduced balance confidence support a falls-risk assessment pathway.

Movement-triggered or positional spinning can justify vestibular assessment as a starting pathway, with diagnosis-specific treatment only after assessment.

The Concierge can align reported needs with Clinics GRP physiotherapy, home physiotherapy, rehabilitation, post-hospital care, balance and falls, vestibular care, therapeutic exercise, older-person nursing, remedial massage, healthy ageing and digital support pathways.
Does the Care Concierge diagnose my condition?
No. It is a service-alignment guide, not a diagnostic or emergency tool. It uses the concerns you select to identify a possible starting service, and Clinics GRP intake must still confirm clinical suitability and any required escalation.
Can the Care Concierge suggest more than one service?
Yes. It provides one recommended starting pathway and may identify up to three supporting services that could work together. Intake confirms whether one service or coordinated care is appropriate.
Where can Clinics GRP services be provided?
Depending on the service, clinical suitability and availability, care may occur at Clinics GRP clinics in Coorparoo, Yeronga or Taigum, at home across current Brisbane and Moreton Bay coverage areas, in community or supported-care settings, or through digital guidance.
