Therapeutic Exercise Program

Exercise and clinical care shaped around your needs. Individually prescribed exercise, movement practice, education and supporting treatment where appropriate, with care adjusted as your needs change.

Your starting point

Start with what matters to you

When a condition, injury or physical problem makes everyday activity difficult, care starts with understanding what has changed and what matters to you.

Your goal might be getting out of a chair more comfortably, walking to a local café, returning to gardening or managing a caring role. The appropriate goal and what is achievable will differ from person to person.

An assessment considers your concern, relevant health information, movement and physical capacity, the activity you want to manage, and the support available to you. Together, you and your clinician decide on a suitable starting point.

Your individual plan

Each part of care has a purpose

Not everyone needs the same exercises or treatments. Each part of the plan should have a reason for being included.

Exercise and movement
Strength or endurance exercise, movement and balance practice, conditioning, or practice of an everyday task. The selection, amount, pace and support are tailored to your needs.
Education and agreed practice
Understand your plan and how to apply it between appointments, with guidance shaped around your circumstances and available support.
Supporting treatment
Treatment, including manual therapy where appropriate, may form part of care. Your clinician explains its purpose, seeks your consent and reviews whether it is useful for the problem being addressed.
Ageing in Full

Care connected to your life

Ageing in Full asks us to keep care connected to the person and the life they want to live. In this programme, that means listening to what matters to you and considering the capacity, choices and support available.

The purpose may be to recover an ability, maintain something you value, improve comfort or tolerance, or find another way to participate. Accepting help or adapting an activity can be a useful part of the plan.

Exercise measures provide information, but we also need to understand what is happening in everyday life. If the approach is not helping, we review the plan with you and consider what needs to change.

Ways to take part

Individual care, a class, or both

The programme can involve individual appointments, the Therapeutic Exercise Class, or a combination. Attending a class is not a requirement.

Individual appointments

Care is shaped around your needs and practical circumstances. Individual care may be considered in clinic, at home, in the community or in supported care, where services are available and appropriate.

A supervised shared session

The Therapeutic Exercise Class provides a shared setting for individual plans. Your exercise, equipment, pace and support may differ from those of the person beside you.

Explore the Therapeutic Exercise Class

Digital support may assist with information, coordination and agreed guidance. It does not by itself mean a remote appointment or class is available.

Review and adapt

Agree the next step together

Review considers your goal, symptoms and tolerance, relevant physical measures, everyday activity and whether the plan remains practical for you.

Your clinician may adjust exercise, treatment, support, setting or goals. You may continue with the current approach, combine different forms of care, consider another programme or conclude supervised participation. There is no single progression everyone must follow.
